How to Login to the LVHN Patient Portal
- Visit the Login Page: Go to the MyLVHN login page at MyLVHN.org.
- Enter Your Credentials: Input your username and password in the designated fields.
- Access Your Account: Click on the “Login” button to enter your account.

How to Sign Up/Register for the LVHN Patient Portal
- Access the Sign-Up Page: Go to MyLVHN.org and look for the “Sign Up Now” button on the homepage.
- Choose Your Method: You can sign up using either a computer or the MyLVHN app:
- On a Computer: Click on “Sign Up Now” and follow the instructions to create your account using an activation code. If you don’t have a code, select “Sign Up Online” and fill out the form.
- On the MyLVHN App: Download the app from Google Play or the Apple App Store. Open the app, click on “Sign Up Now,” and follow the prompts. If you lack an activation code, select “Sign Up Online” and complete the required information.
- Activation: After completing the sign-up process, you may receive a text or email to activate your account. Follow the instructions provided.
