How to Sign Up For the SOMC Patient Portal

To sign up for the SOMC Patient Portal, you can follow these steps:

1. Visit the SOMC Patient Portal sign-up page.
2. Enter your last name, first name, date of birth, and either your medical record number or the last four digits of your Social Security number.
3. Provide a valid email address and confirm it.
4. Create a username that meets the following criteria:
   • At least 8 characters long but no more than 99 characters.
   • Includes at least 2 numeric characters.
5. Create a password that meets the following criteria:
   • At least 8 characters long but no more than 99 characters.
   • Includes at least 1 numeric character, 1 uppercase character, and 1 lowercase character.
6. Confirm your password.
7. Submit your registration details to create your account.
